Prior to participation in athletics at UW-River Falls, all student-athletes are required to have the necessary medical forms on file with the athletic training staff. All medical forms are required to be updated and on file with the athletic training staff each year by August 1.

*A note to student-athletes taking prescription medications. Certain prescription medications are classified as banned substances by the NCAA (i.e. ADHD medications). It is your responsibility to report all medications to the athletic training staff. If you are taking prescription medications that are classified as banned substances, the NCAA Medical Exemption Form must be completed by your prescribing physician and kept on file with the athletic training staff.
**Effective 8/1/2010: all NCAA Division III student-athletes must provide evidence of insurance that must have coverage of at least $90,000 which includes coverage for athletically related injuries. No student-athlete will be allowed to participate in practice or competition until such evidence of current insurance is on file with the athletic training staff. If you are unsure of your coverage, you should contact your insurance company immediately.
^Effective 8/1/2013: all NCAA Division III student-athletes must provide evidence of their sickle cell trait status or sign a testing waiver prior to participation in intercollegiate athletics. No student-athlete will be allowed to participate in practice or competition until such evidence is on file with the athletic training staff.
